Please download the form below and fill it out before your appointment time. If you do not have a printer or cannot fill out the form beforehand, forms will be available at your appointment. Shortly after scheduling, you’ll receive an email confirming your appointment time. Please bring your ID, insurance card if applicable, and completed form in with you. You will need to remain in our office for 15 minutes after your appointment for monitoring. We will have our cafe open for smoothies and coffee drinks while you wait!
